Apply for NCID & ePass. An NCID is a secure online user ID and password you set up to access NC Medicaid. You will need an NCID to move to the next step of the Medicaid Application Process, and log into your new ePass Account.

Apply for NC Medicaid In Person
Find your local Department of Social Services (DSS) using this link to the DSS directory or call the NC Medicaid Contact Center at 1-888-245-0179 to find the nearest office. In-Person applications are accepted at DSS offices for; walk-in, scheduled in-person appointment, or scheduled phone interview. Scheduling an in-person appointment or phone interview is usually the fastest option.
Print and fill in the DMA-5200 application before your appointment with DSS for faster service. DSS also provides pre-printed forms in their offices. Single adults without any health care coverage from their employer, submit the “short form” application (DMA-5201).
